Vision Quest: Caregiver Involvement
The Vision Quest team places importance on family and caregiver involvement. We believe frequent communication and follow-up are essential to exceptional care. Caregivers are included in the following:
- Assessment phase
- Setting goals
- Interdisciplinary team meetings
- Discharge planning
- Follow-up care
